I like making graphics, so just fill out a form if you would like me to make you one.
Here are some examples of my work:

Signature Banner Size: 600 by 220 should be fine.
Icons? One should be enough.
Name of Model: Avril Lavigne. (Please no pictures with the pink highlights.)
